Forms

• Order Form-Use this form to place your orders. Once submitted, a printable copy will show up on your screen, and the order will be e-mailed to purchasing and to your shop's e-mail account for your records.
• Emergency Purchase Order Form-This form is for emergency after-hours use only. Once the form is submitted, you will be provided with a PO number and a printable copy of the form on your screen. The order will be e-mailed to the central purchasing agent, to your shop's e-mail account, and to Shari so that she can enter in the next emergency PO number. In the event that you require multiple emergency po #s in one night, for now just fill out separate forms for each order, and append an "A", "B", "C", etc. after the purchase order number that is provided to you, since the form will not automatically bring up the next PO in line. Make sure you contact the central purchasing agent to let them know that there were multiple orders, and which ones correspond to "A", "B", "C", etc, since this information will not be transmitted to them. Whenever you use an emergency PO, double-check the next day to make sure that central purchasing received the emergency order information successfully.
• Absence Request Form-This is an electronic version of the Absence Request Form; please note that this is a request only, and it must be approved by your manager before taking effect. In Peoria, please print this out and submit to Dave as you always have after filling it out. Bloomington, Galesburg, and Macomb may use this to e-mail the form to your respective managers, who will then let you know if your vacation is approved.
